Key Responsibilities\n

• Conduct comprehensive assessments to better understand and meet the needs of clients seeking mental health support.

• Maintain open lines of communication with supervisors, support staff, and family members to discuss individual progress and emerging needs.

• Develop and provide a tentative schedule for appointments, home visits, and any necessary observations to monitor client advancement.
\n
• Coordinate necessary home visits, collaborating with team members for in-home observations and data collection to ensure thorough care.
\n
• Evaluate and ensure the efficacy of the treatment plans and program implementation for each client.
\n
• Engage in interdisciplinary team meetings, contributing insights and collaborating on client care strategies.
\n
• Prepare and complete monthly and quarterly progress notes accurately documenting client milestones and interventions.
\n
• Organize and facilitate training for staff on the program plans and documentation requirements to uphold the quality of care.
\n
• Provide ongoing support through follow-up interviews and additional home visits to monitor the needs and progress of clients.
\n
• Keep accurate records that facilitate easy access to documentation for clients, families, and authorized stakeholders.
\n
• Serve as a liaison between the organization and the wider mental health community to foster connections and enhance service delivery.

• Assist in the development of training materials that effectively improve program delivery and documentation standards.

Patient Population / Client Focus\n

This role primarily serves a diverse clientele, including adults and youth, who may be dealing with a range of mental health concerns such as anxiety, depression, trauma, and substance misuse. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Possession of an active and unrestricted LPC license in the state of Texas.
