Key Responsibilities\n

• Guide patients on how to access vital community resources, including vocational rehabilitation services, support groups, and housing assistance to aid their recovery.

• Educate patients and their families on community resources and post-discharge services, enhancing their capacity for independent living post-rehabilitation.

• Engage in proactive discharge planning activities, ensuring seamless transitions for patients returning home or moving to other care environments.
