Substance Use Disorder Counselor
Apex Home Health, Hospice & Home Care
Role: Substance Use Disorder Counselor Responsibilities: Provide individual and group counseling, conduct assessments, screenings, and treatment planning, deliver case management, coordinate community support services, maintain clinical documentation, support clients’ recovery and stability, collaborate with multidisciplinary teams and community partners, and participate in supervision and professional development. Qualifications: Valid Wisconsin SAC-IT, SAC, or CSAC credential, associate’s degree in human services or related field (bachelor’s preferred), knowledge of addiction and recovery principles, experience with justice-involved populations is a plus, valid driver’s license, and reliable transportation. Key Skills: Strong counseling, case management, documentation, teamwork, and community collaboration.
