Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage patients' mental health and psychosocial issues effectively.

• Conduct initial patient intakes, performing diagnostic and functional evaluations, and make informed recommendations for treatment plans based on findings.

• Deliver brief, focused therapeutic interventions to patients in need, considering individual treatment goals and preferences.

• Provide ongoing assessments and follow-ups to monitor patients' progress and modify treatment plans as needed.

• Assist in creating personalized relapse prevention strategies to help patients maintain stability in their lives.

• Identify 'at-risk' patients and develop proactive plans to mitigate the worsening of their conditions.

• Coordinate care by linking patients with relevant community resources and other treatment providers, with their consent.

• Maintain accurate and up-to-date clinical documentation of patients’ progress and treatment plans within electronic health records.

• Advise on behavioral modifications and guide patients towards healthier lifestyle choices through education and support.

• Participate in coordination meetings with clinic management and interdisciplinary team members to enhance the delivery of behavioral health services.

Qualifications\n

Compensation and Benefits\n

The anticipated salary for this position ranges from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
