Key Responsibilities\n

• Conduct thorough psychosocial assessments to establish the behavioral health needs and formulate comprehensive care plans for patients.

• Utilize knowledge of psychopathology to inform treatment modalities and tailor approaches for individual patient circumstances, including those facing end-of-life challenges.

• Ensure that treatment plans are consistently evaluated and modified based on patient progress and ongoing assessments.
\n
• Collaborate closely with physicians and other healthcare professionals to discuss clinical observations, address behavioral health needs, and optimize patient outcomes.

• Provide education to patients and families regarding mental health issues, including grief and loss, facilitating effective coping strategies.
\n
• Act as a liaison within the community, connecting patients with necessary resources and support systems outside of our medical center.

Patient Population / Client Focus\n

This role primarily serves adult patients in hospice care, often dealing with complex emotional, psychological, and social issues surrounding end-of-life decisions. A strong foundation in cultural competency is essential to provide compassionate care that respects diverse backgrounds and life stages, from younger adults to the elderly.

Qualifications\n

• A Master's Degree in Social Work (MSW) from an accredited program.

• Licensed as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.
\n
• Minimum of 2 years of direct experience in social work, especially within psychiatric or behavioral health settings.

• Current certification in the safe management of disruptive and assaultive behavior.
\n
• Demonstrated flexibility and adaptability to meet the changing needs of patients and the healthcare setting.

Compensation and Benefits\n
