Key Responsibilities\n

• Utilize your academic and practical experience to effectively conduct independent psychosocial assessments, identifying patients' needs and crafting tailored intervention strategies.

• Collaborate with patients to build individualized, patient-centered treatment plans that prioritize their unique circumstances, preferences, and treatment goals.

• Implement evidence-based behavioral interventions, such as solution-focused therapy, motivational interviewing, and behavioral activation, adapted to the specific needs of each patient.

• Coordinate with fellow clinicians, psychologists, and primary healthcare providers to foster a holistic, integrated care approach for comprehensive support.

• Regularly track treatment efficacy, documenting patient progress and adjusting care strategies as necessary during both virtual and in-person sessions.

• Assist in the management of psychotropic medication as prescribed, monitoring adherence and side effects, and communicating effectively with medical providers regarding treatment effectiveness.

In this role, you will primarily serve patients receiving hospice care, often dealing with chronic illnesses, terminal diagnoses, and end-of-life transitions. Qualifications\n

• Master’s degree in Social Work from an accredited program.
