This role is conducted entirely through a telehealth setting. Key Responsibilities\n

• Conduct thorough assessments of patients’ psychosocial needs to establish personalized treatment plans that may include short-term counseling and crisis intervention strategies.

• Assist clients in navigating and completing necessary documentation related to entitlements and supplemental insurance to facilitate their access to essential benefits.
\n
• Provide support in accessing prescription cost assistance programs, demonstrating a strong advocacy role in eliminating barriers to medication access through community and pharmaceutical resources.

• Actively advocate for patients in communications with community agencies, representing their interests and ensuring they receive comprehensive support.
\n
• Maintain meticulous and up-to-date patient records, preparing reports and other documentation to ensure compliance with regulatory and organizational requirements.

• Coordinate necessary transportation assistance for patients, ensuring they are able to attend appointments and receive care.
\n
• Participate in clinical team meetings and case conferences to collaborate on patient care strategies and improve service delivery.

Patient Population / Client Focus\n

In this role, you will work with a diverse clientele, including individuals facing mental health challenges such as anxiety, depression, trauma, and a range of emotional and behavioral issues. You will engage with clients from varying backgrounds, ensuring culturally competent care tailored to their individual circumstances and needs.

Qualifications\n

• A Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required for this position.

• Possession of an active and independent license is necessary, with a preference for Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two (2) years of professional experience in mental health counseling is required, demonstrating a solid foundation in psychotherapeutic practices.

Preferred Experience\n

Ideal candidates will have experience working in a telehealth environment, with a proven ability to effectively conduct sessions virtually. Familiarity with electronic health record (EHR) systems and patient management software is highly desirable, ensuring seamless interaction with patient data and case documentation.

Compensation and Benefits\n
