Key Responsibilities\n

• Conduct comprehensive, ongoing psychosocial assessments for patients and their families to identify concerns, strengths, needs, and any issues related to spiritual and cultural values.

• Develop and periodically revise individualized treatment plans that address high-risk psychosocial needs, while focusing on leveraging strengths and minimizing deficits.
\n
• Facilitate educational and therapeutic group sessions that promote healing and resilience among participants, documenting patient goals and responses to group activities.
\n
• Attend regular treatment-planning meetings to provide and receive feedback, contribute insights regarding patient progress and challenges, and document updates on respective treatment plans.

• Act as a liaison between patients and community services to ensure effective aftercare arrangements that enhance continuity of care.

Patient Population / Client Focus\n

This role primarily serves a diverse patient population at our community health clinic, focusing on individuals facing challenges such as mental health issues, substance use disorders, and socio-economic barriers to wellness. As a clinical social worker, you will play a key role in addressing these multifaceted challenges, helping clients navigate their obstacles through teamwork and an understanding of cultural competencies.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active California license as a Clinical Social Worker (LCSW).

Compensation and Benefits\n

This full-time position offers a competitive annual salary ranging from USD 120,000 to 130,000 , commensurate with your experience and caseload.
