Title APP LakeView Family Care (Suite A) NP or PA-C – Paw Paw, MI Location BLH Bronson LakeView Hospital Overview Bronson LakeView Family Care - Paw Paw (Suite A) in Paw Paw, MI is seeking to hire a Nurse Practitioner or Physician Assistant to join their busy established Family Medicine Practice. Position Highlights See full spectrum family practice patients Candidate will work alongside two physicians and two APPs No OB or inpatient medicine opportunity Hospital‑employed, Full‑time outpatient only position Team Approach with Ambulatory Care Manager, clinical pharmacy support, social worker, as well as nurse triage. As a Rural Health practice, appointment schedule will allow early starts at 7:30 am or late finish at 7 pm at least one day a week. Patients have access to an onsite pharmacy, laboratory and imaging. Nurse Triage service is available after hours and on weekends; after‑hours call volume is very low. APP will provide call coverage one evening during the week and share weekend and holiday coverage with 17 additional family care providers. Qualifications Successful completion of an accredited Physician Assistant or Nurse Practitioner Program Current unrestricted or pending Licensure in the state of Michigan State DEA license to prescribe medications (for Physician Assistants) Current prescriptive authority and collaborative agreement with oversight physician(s) in place Ability to maintain all requirements for medical staff privileges Strong communication and interpersonal skills Commitment to providing compassionate and quality patient‑centered care Responsibilities Clinical Care Perform comprehensive history and physical examination Interpret laboratory and radiologic tests, correlate subjective and objective data, and formulate diagnoses Develop treatment plans under physician direction within scope of practice Provide care in outpatient settings; support inpatient situations as needed Demonstrate required competencies specific to the patient population served Care Coordination (Concurrent Clinical Management) Coordinate care with the multidisciplinary team to facilitate implementation of patient progress towards outcomes, including discharge or transfer of care Act as a clinical resource for the team, assertively coordinating among team members Identify common patient complications and utilize appropriate preventative measures Assist in the development and integration of quality measures and evidence into clinical practice Utilization Review Perform initial comprehensive assessment to determine if a higher intensity of service or transfer to a hospital setting is required Apply knowledge of regulatory and third‑party payer criteria to guide decisions Facilitate timely discharges or transfers to the appropriate level of care Clinical Documentation Evaluate lab, EKG, and imaging studies Complete appropriate documentation to support billing, medical care, orders, and progress notes Be available for any nursing questions or to evaluate status changes for patients Administrative and Technical Skills Utilize word processing, spreadsheet, presentation programs, and other software relevant to the job Communicate clearly and effectively, both verbally and in writing, using SBAR technique Communicate with internal customers (physicians, multidisciplinary team, finance, management) and external customers (patients, third‑party payers, community agencies) on a daily basis, often simultaneously Schedule Shift: Variable Time Type: Full time Scheduled Weekly Hours: 40 #J-18808-Ljbffr Provider Careers at Bronson Medical Group
