Key Responsibilities\n

• Conduct thorough assessments and create individualized treatment plans aimed at addressing the unique needs of each client.

• Collaborate with multidisciplinary teams to develop crisis intervention plans, medication management strategies, and behavioral support guidelines.

• Maintain up-to-date and accurate clinical documentation in accordance with regulatory standards, ensuring timely completion of progress notes and treatment updates.

• Utilize evidence-based screening tools to effectively evaluate and monitor clients’ progress and needs.

• Participate actively in licensing and accreditation processes while serving as a critical liaison between clients and relevant service systems, including state agencies and community partners.

• Engage in community outreach to promote programs and identify new partnerships that enhance service offerings and funding opportunities.

• Seek continual personal and professional advancement by attending workshops, conferences, and statewide meetings related to mental health.

Patient Population / Client Focus\n

In this role, you will engage with a diverse population of clients requiring varying levels of mental health support. This includes individuals experiencing emotional distress, those recovering from substance abuse, and clients with developmental disabilities. Qualifications\n
