Home Health Physical Therapist

Addus Family Company seeks a compassionate Physical Therapist to deliver high-quality, home-based rehabilitative care for seniors and individuals with disabilities. In this role, you will assess functional status, develop and implement individualized treatment plans, and provide hands-on therapy to improve mobility, safety, and independence. You will collaborate closely with nurses, caregivers, and families to coordinate care, educate on home exercise programs, and monitor progress.
