Key Responsibilities\n

• Conduct comprehensive assessments to determine the counseling needs of clients, including substance abuse evaluations using DSM criteria for accurate diagnosis.

• Prepare and implement individualized treatment plans that incorporate counseling interventions for clients diagnosed with substance use disorders.
\n
• Develop discharge and continuity of care plans that facilitate clients’ transition to the most appropriate level of care, ensuring ongoing support and resources.
\n
• Document all interactions, assessments, treatment plans, and progress notes in clients’ clinical records accurately and timely.
\n
• Maintain a manageable caseload, providing case management services and adhering to treatment planning and clinical charting standards.
\n
• Coordinate support services for clients and their families, documenting all provided services in clinical records.
\n
• Engage with referral sources, including legal entities, educational institutions, and employers, to provide comprehensive care for clients.
\n
• Facilitate crisis interventions as necessary, employing a collaborative multi-disciplinary team approach to address urgent client needs.

This role directly influences the lives of individuals grappling with substance use and co-occurring disorders. You will work with a diverse clientele, including adults transitioning from acute care settings to outpatient treatment, ensuring they receive consistent support tailored to their unique circumstances. Qualifications\n

• A Master’s Degree in Social Work from an accredited institution, along with an active LCSW license in New York.

• Strong familiarity with the DSM and ASAM criteria for diagnosis and treatment planning.

• Proficient in documenting clinical records and treatment plans in compliance with organizational standards and applicable laws.

• Exemplary professional demeanor with the ability to establish therapeutic boundaries and engage effectively with clients from varied backgrounds.

• Possess excellent organizational and time-management skills to manage a caseload effectively.

Preferred Experience\n

Prior experience in a community health clinic or similar setting focusing on addiction treatment is preferred. Familiarity with various therapeutic modalities and demonstrated competency in delivering culturally responsive therapy will be advantageous.

Compensation and Benefits\n

We offer a competitive salary ranging from USD 70,000 to 100,000 annually, commensurate with experience and educational background. Our comprehensive benefits package includes:
