Key Responsibilities\n

• Conduct thorough assessments to identify clients' social, emotional, and environmental challenges, ensuring a personalized approach to intervention.

• Develop and implement comprehensive service plans that outline specific goals, recommended interventions, and methods for tracking client progress over time.

• Provide individual and family counseling that addresses a wide range of life challenges, such as navigating grief, dealing with chronic illnesses, and managing daily stressors.

• Collaborate with clients to craft tailored service plans, ensuring connections to essential community resources, such as housing support services, mental health care, and legal assistance.

• Deliver crisis intervention services, including risk assessments and safety plans, to effectively address urgent concerns and ensure client safety.

• Engage proactively with community agencies and organizations to coordinate services and advocate tirelessly for client needs.

• Participate in community outreach to promote awareness of our agency’s services and foster stronger community ties.

• Contribute to team discussions and case conferences, sharing insights and strategies to enhance client support.

Patient Population / Client Focus\n

The LCSW will focus on diverse client populations, primarily composed of individuals and families experiencing acute emotional distress, chronic illness, or situational crises related to end-of-life care. Our client base includes those facing significant life transitions and social challenges, such as terminal illness, financial instability, and lack of access to appropriate medical resources. Your expertise will play a vital role in helping clients navigate these complex situations with dignity and support.

Qualifications\n

• A Master's degree in Social Work from an accredited university is mandatory.

Compensation and Benefits\n
