Key Responsibilities\n

Your duties as a Licensed Professional Counselor will include:

• Collaborating with primary care providers to identify, treat, and manage a variety of mental health and psychosocial issues.

• Conducting thorough patient intakes, performing diagnostic and functional evaluations to formulate tailored treatment plans.

• Delivering brief, focused interventions for patients seeking mental health assistance while maintaining productivity metrics for billable services.
\n
• Providing recommendations to primary care providers on treatment goals, assisting with referrals to specialized mental health services when appropriate.
\n
• Initiating follow-up care to evaluate patient progress and adjusting treatment plans as necessary.
\n
• Developing relapse prevention strategies and monitoring at-risk patients to avert deterioration of their conditions.

• Coordinating behavioral health care services, ensuring efficient delivery of services both within our clinic and externally, with patient consent.
\n
• Documenting all patient interactions and progress within clinical records to ensure continuity of care.
\n
• Engaging in team discussions to enhance behavioral services and outcomes.
\n
• Providing consultation to clinic management regarding behavioral health program evaluation and potential improvements.

• Supporting patients in adhering to medical treatments as prescribed, offering practical coping strategies for challenges such as medication side effects.

Patient Population / Client Focus\n

Qualifications\n

To qualify for this position, candidates must possess:

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

• Current LPC (Licensed Professional Counselor) License to practice in the state of Connecticut.
