Position Overview

North West Physiotherapy is currently seeking a dedicated and innovative Full-Time Home Care Physical Therapist to join our compassionate team in Bonita Springs, Florida. In this role, you will be responsible for evaluating and treating patients in the comfort of their homes, working closely with families, physicians, and other healthcare professionals to develop and implement effective treatment plans that improve patient mobility and quality of life.

Work Environment

This position offers a full-time, onsite working environment, allowing therapists to engage directly with patients in their homes across Bonita Springs. The typical work schedule is Monday to Friday, from 9 AM to 5 PM, providing a structured and supportive environment for our staff to thrive. You will work autonomously within the community, travel between locations, and bring your expertise directly to patients in need.

Key Responsibilities

• Promptly evaluate patients within 24 hours of receiving a physician referral, documenting any delays and their justifications in medical records to ensure continuity of care.

• Develop comprehensive treatment plans based on evaluations to address the specific needs and goals of each patient, obtaining necessary approvals from referring physicians to initiate services.

• Implement treatment sessions as prescribed, utilizing evidence-based techniques and procedures tailored to each patient's condition and individual progress.

• Supervise Physical Therapy Assistants in delivering patient care, ensuring compliance with state practices and standards while fostering a collaborative team approach.

• Communicate effectively with the healthcare team regarding changes in patient conditions, progress updates, and any issues that may arise, facilitating a shared understanding of treatment goals.

• Participate in multidisciplinary meetings, such as Patient Care Conferences and Rehabilitation Conferences, to ensure coordinated and effective patient management.

• Conduct in-service training sessions for facility staff to enhance their knowledge and skills in restorative and rehabilitative practices.

• Maintain meticulous documentation of evaluations, treatment notes, and patient progress, adhering to the guidelines set forth by the PT Board and third-party payers.

• Instruct nursing staff and family members on caregiver training and maintenance programs, preparing them for discharge and enhancing the patient's independence and functional capacity.

• Assist in discharge planning, collaborating with the care team to identify and facilitate necessary durable medical equipment orders, such as walkers or grab bars, to support patients' mobility post-therapy.

• Proactively report and ensure the maintenance of department equipment, contributing to a safe and effective therapeutic environment.

Patient Population / Client Focus

As a Home Care Physical Therapist, you will primarily work with a diverse patient population, including those recovering from surgery, managing chronic conditions, or rehabilitating from injuries. Your ability to provide tailored interventions in their own environment will be instrumental in fostering their recovery journey and helping them regain independence.

Qualifications

• Graduation from a reputable Physical Therapy program with a Bachelor's degree in Physical Therapy.

• Valid Florida State Licensure as a Physical Therapist (PT).

• Minimum of two years of clinical experience in a relevant healthcare setting.

• Demonstrated ability to develop and implement effective treatment plans based on evolving patient needs.

• Strong interpersonal and communication skills for effective collaboration with patients, families, and interdisciplinary team members.

• Exceptional organizational skills with the capability to manage multiple patients and treatment sessions effectively.

• A proactive approach with a drive for professional growth and commitment to delivering high-quality patient care.

Preferred Experience

• Experience in outpatient orthopedics, encompassing exercise therapy, manual therapies, and sports rehabilitation is highly desired.

• Proficiency in using electronic medical records (EMRs) and familiarity with documentation standards for various healthcare payers.

Compensation and Benefits

We offer a competitive salary range of $90,000 to $100,000 per year, based on experience and expertise, along with a comprehensive benefits package that may include healthcare options, retirement plans, and paid time off.
