Work Environment\n

This position primarily allows for remote work, enabling you to connect with clients and their families from the comfort of your home. Our hospice center provides a collaborative work atmosphere where you will interact closely with healthcare professionals, caregivers, and families. The typical work hours are Monday through Friday, from 8:00 AM to 4:30 PM, with some flexibility required for evening or weekend shifts based on client needs.

Key Responsibilities\n

• Conduct thorough assessments to identify the psychosocial needs of seniors, considering emotional, social, and mental health factors.

• Collaborate with clients and families to create personalized care plans that address individual goals, preferences, and needs.

• Provide counseling and emotional support, assisting clients in navigating significant life transitions such as loss or illness.

• Advocate for clients' rights, ensuring access to necessary community resources and services while enhancing their quality of life.

• Work closely with an interdisciplinary team that includes nurses, doctors, and other support staff to coordinate comprehensive care.

• Conduct regular check-ins with clients to monitor their well-being, assess living conditions, and proactively identify any safety concerns.

• Facilitate family meetings to educate and inform all members about care strategies and available support systems.

• Maintain meticulous records of client interactions, care strategies, and overall progress, utilizing electronic health record (EHR) systems.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
