Key Responsibilities\n

• Conduct assessments to understand clients' needs and develop personalized treatment plans for individuals experiencing a range of mental health challenges.

• Facilitate individual, group, and family therapy sessions, employing evidence-based practices to help clients achieve their personal goals.

• Collaborate with multidisciplinary teams, including nurses, physicians, and other social workers, to provide integrated care and enhance overall patient outcomes.

• Provide on-call crisis intervention support as needed, ensuring that clients receive immediate assistance during critical moments.

• Document all client interactions and maintain accurate records in alignment with regulatory requirements and best practices, ensuring timely report submissions and data entry.

This role focuses primarily on clients residing in a hospice center who are navigating complex emotional and mental health issues related to terminal illnesses. Qualifications\n

• A Bachelor’s Degree in Social Work (BSW) is required; a Master’s Degree in Social Work (MSW) is preferred.

• A valid state license to practice as a Licensed Clinical Social Worker (LCSW) in Colorado.

• At least 2 years of relevant clinical experience, preferably in hospice, palliative care, or a community-based setting.

Compensation and Benefits\n

The annual salary for this position ranges from USD 70,000 to 85,000, commensurate with experience.
