Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand clients’ mental health issues, strengths, resources, and family dynamics.

• Provide individual, family, and group therapy tailored to the needs of hospice patients, utilizing evidence-based therapeutic modalities such as cognitive-behavioral therapy and solution-focused therapy.

• Develop, implement, and monitor individualized treatment plans, ensuring that all interventions are aligned with the goals of the patient and their families.

• Work collaboratively with a multidisciplinary team to create integrated care strategies that address both physical and emotional comfort for patients approaching end-of-life.

• Engage in regular case management meetings to review patient progress and adjust treatment plans as necessary.

• Provide crisis intervention and safety planning when required, ensuring immediate support is available for patients and their families during acute emotional distress.

• Maintain accurate and timely documentation of client encounters per ethical and legal standards, contributing to the overall quality of care and organizational accountability.

• Facilitate psychoeducation classes and support groups aimed at equipping families with resources and coping strategies during the hospice care process.

Patient Population / Client Focus\n

This position focuses on adults and families coping with terminal illnesses. As an LCSW, you will provide critical emotional support, helping patients navigate their feelings and thoughts about death and loss. Your work will directly contribute to enhancing the wellbeing of patients in hospice, as well as supporting their loved ones through grief and bereavement, ensuring that they feel heard and supported every step of the way.

Qualifications\n

• Master's degree in Social Work from an accredited program.
