Work Environment\n

This position is remote and will focus primarily on hospice care, enabling social workers to assist clients from the comfort of their homes. While you’ll primarily work from home, you will regularly engage with clients in their living environments, conducting assessments, and facilitating multi-disciplinary team meetings. Key Responsibilities\n

• Perform in-depth psychosocial assessments of seniors, incorporating emotional, mental, and social factors that affect their well-being.

• Develop and implement personalized care plans in close consultation with clients and their families, reflecting their specific goals, preferences, and values.

• Provide individual counseling and emotional support, equipping seniors and their families with coping strategies as they navigate life transitions, such as illness or loss.

• Act as an advocate for clients’ rights, helping them access critical resources and services within the community.

• Collaborate with an interdisciplinary team—including nurses, therapists, and counselors—to optimize care and coordinate all aspects of service delivery.

• Conduct regular in-home visits to monitor clients' situations, addressing safety issues, living conditions, and any evolving psychosocial needs.

• Facilitate open communication among family members, aiming to resolve conflicts and foster a team approach to care.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

Compensation and Benefits\n

This full-time position offers a competitive salary ranging from $60,000 to $100,000 annually, commensurate with experience and qualifications.
