Work Environment\n

This role is designed for a remote work setting, allowing for flexibility in delivering services. You will primarily connect with clients through virtual sessions, but you will also conduct occasional in-person assessments and visits to ensure a continuous support system. The work hours are typically Monday to Friday from 8:00 AM to 4:30 PM, with potential evening or weekend hours depending on the clients' needs, ensuring you can adapt to various situations.

Key Responsibilities\n

• Perform in-depth assessments of seniors' psychosocial well-being, evaluating emotional, mental, and social factors affecting their quality of life.

• Develop individualized care plans in collaboration with clients and their families, incorporating their personal goals and preferences.

• Provide counseling and emotional support, guiding clients through life transitions such as bereavement, health changes, or relocation.
\n
• Act as an advocate for clients, ensuring they have access to necessary community resources and services, including healthcare and social services.
\n
• Collaborate with multidisciplinary teams—including nurses and healthcare professionals—to coordinate care and enhance service delivery.

• Conduct regular visits to monitor client well-being, evaluating living conditions and identifying any potential safety issues.

• Facilitate open communication among family members to resolve conflicts and ensure cohesive care delivery.

• Educate clients and families about community resources, support groups, and programs that promote social engagement.

• Maintain meticulous documentation of all assessments, care plans, notes on client interactions, and progress to ensure compliance with regulations.

• Stay updated on best practices in social work, attending workshops and training to enhance skills and knowledge.

Your primary focus will be on seniors experiencing various challenges related to aging, including physical health issues, emotional distress, and social isolation. By establishing trusting relationships, you will guide them toward solutions that foster independence and empower their decision-making processes.

Qualifications\n

• Master's degree in Social Work from an accredited institution is required.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of clinical social work experience, with a preference for experience in geriatric or community health settings.

• In-depth knowledge of social work practices, aging-related issues, and available community resources for seniors.

Compensation and Benefits\n
