Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments, identifying the unique needs of each patient and formulating individualized treatment plans based on these assessments.

• Provide tailored short-term and long-term therapeutic interventions, applying evidence-based approaches suited to the diverse healthcare setting.

• Coordinate effectively with hospital staff, outpatient providers, and community resources to manage patient care and discharge planning seamlessly.

• Assist patients and their families in navigating the complexities of healthcare systems, including helping them access insurance benefits, financial assistance, and other social services they may require.

• Facilitate support groups and psychoeducational workshops, engaging patients in mental health discussions to promote community knowledge and resilience.

• Address and advocate for social determinants of health that impact recovery, which may include assisting patients with housing, employment, and accessing healthcare services.

Compensation and Benefits\n
