Work Environment\n

As a remote position, this role offers flexible work-from-home arrangements, removing geographical barriers while allowing you to provide vital services directly to our clients. While the core working hours are Monday to Friday, from 8:00 AM to 4:30 PM, occasional evening or weekend hours may be necessary to meet our clients' changing needs. Flexibility is essential for accommodating client visits and ensuring continuous care, and on-call support may also be required for urgent situations.

Key Responsibilities\n

• Conduct comprehensive evaluations to assess the emotional, mental, and social needs of senior clients, utilizing standardized assessment tools to inform your recommendations.

• Collaborate closely with clients, their families, and interdisciplinary care teams to develop individualized care plans that align with clients' goals and preferences.

• Provide ongoing counseling and emotional support to clients during significant life transitions, such as health changes or bereavement, guiding them through challenges with empathy and expertise.

• Advocate for clients’ rights and help them navigate access to community resources, ensuring they receive comprehensive support and services.

• Maintain thorough documentation of client interactions, assessments, and care plans to ensure continuity of care and compliance with healthcare regulations.

• Facilitate effective communication among family members and caregiving teams, resolving conflicts and enhancing collaboration to achieve optimal client outcomes.

• Engage in continuous learning to stay informed about best practices, treatment modalities, and regulatory standards in social work and mental health.

Patient Population / Client Focus\n

The primary focus of this role is our senior client base, which includes individuals who are grappling with various mental health challenges exacerbated by aging. Many clients may experience issues such as depression, anxiety, grief, or adjustment disorders. You will also be instrumental in empowering not just the clients but their families, providing the support needed for caregivers who may also face stress or burnout.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited university.

• Current and valid LCSW licensure in the state of Pennsylvania.

• Minimum of two years of experience in social work, with a preference for candidates who have worked in geriatric settings or community health environments.

• Strong understanding of aging-related issues and the community resources available to support this demographic.

• Excellent counseling, assessment, and problem-solving skills, with a genuine compassion for helping others.

• Exceptional communication and interpersonal skills to foster relationships with diverse clients and engage effectively with multidisciplinary teams.

• Proficiency in electronic health records (EHR) systems and other relevant software applications.

Compensation and Benefits\n

The salary for this position ranges from USD 90,000 to 100,000 yearly.
