Key Responsibilities\n

• Conduct thorough assessments of clients' mental health needs to develop tailored treatment plans.

• Facilitate continuous communication among interdisciplinary team members, including supervisors and other healthcare professionals, to ensure cohesive client-focused care.

• Prepare a tentative schedule for client appointments, home visits, and observational tasks to monitor client progress effectively.

• Coordinate home visits and in-home observations while conducting comprehensive chart reviews, staff interviews, and data analysis to meet clients' individual requirements.

• Ensure the effective implementation of treatment programs, adapting as necessary to achieve positive outcomes.

• Participate in Interdisciplinary Team Meetings to review client cases and share insights on treatment effectiveness.
\n
• Document progress notes monthly and quarterly, capturing essential client progress information to inform ongoing treatment.
\n
• Organize and facilitate training sessions to ensure team members are proficient in implementing program plans and maintaining documentation standards.

• Provide essential follow-up support through interviews, home visits, and other forms of outreach to address ongoing client needs effectively.

• Act as a liaison between clients and the wider mental health community to foster partnerships that promote service expansion and developmental goals.

You will provide individual and family therapy primarily focusing on adults and youth, addressing a variety of concerns including trauma, substance misuse, and relational challenges. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, Social Work, or a related field.

• An active and unrestricted Professional Counselor license in the state of Colorado.
