Key Responsibilities\n

• Conduct comprehensive assessments to determine the specific needs of patients, utilizing various assessment tools and techniques to inform treatment planning.

• Engage in effective communication with supervisors, healthcare team members, family members, and guardians, ensuring that all parties are aware of the patient’s progress and evolving needs.

• Create and maintain a schedule of appointments, home visits, and other observations that facilitate ongoing monitoring and support for individuals.

• Coordinate and conduct home visits for client assessments, including observations and data collection to evaluate treatment efficacy and adherence.
\n
• Implement program plans in accordance with clinical guidelines, ensuring that therapeutic interventions are executed effectively to support patient outcomes.
\n
• Participate actively in Interdisciplinary Team Meetings and other relevant discussions to enhance collaborative care planning.
\n
• Document patient progress through monthly and quarterly notes, capturing changes in behavior, emotional status, and treatment response.
\n
• Organize training sessions to ensure that team members are well-versed in program implementation and documentation standards, enhancing the overall competency of the staff.

• Provide follow-up support through interviews, observations, and home visits, responding proactively to the ongoing needs of clients.
\n
• Maintain accurate and organized records to facilitate easy access for all authorized personnel and stakeholders.

• Act as a liaison between patients and the broader mental health community, promoting resources and support for developmental goals.

• Contribute to the evolution of training materials and procedural guidelines to improve service delivery and client engagement.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor in New Jersey.

• Specialization in areas such as family therapy, trauma, or substance misuse is highly desirable.
