Key Responsibilities\n

• Conduct comprehensive assessments and ongoing evaluations to address the individual needs of clients, ensuring tailored therapeutic interventions.

• Maintain clear and consistent communication with supervisors, care team members, families, and other stakeholders to discuss client progress and coordinate care plans.

• Create and manage a structured appointment schedule for consultations and follow-up visits, optimizing the delivery of client-centered services.

• Coordinate home visits, as needed, to provide in-home observations, conduct chart reviews, and engage in data analysis to enhance program effectiveness.

• Implement and monitor the effectiveness of individualized program plans, ensuring they meet established therapeutic goals.

• Participate actively in Interdisciplinary Team Meetings to foster collaboration and enhance multidisciplinary care approaches.

• Document and complete monthly and quarterly progress notes that reflect client advancements and any modifications to treatment plans.

• Organize training sessions for team members to ensure competency in implementing program plans and correct documentation practices.
\n
• Engage in follow-up sessions, home visits, and observations to provide ongoing support tailored to individual client needs.

• Manage and maintain detailed records to ensure that important client information is accessible to authorized personnel as needed.

• Act as a liaison between clients, families, and the mental health community to address developmental and treatment goals effectively.

• Contribute to the development of training materials and procedures aimed at improving program offerings and enhancing client outcomes.

Qualifications\n

• A Bachelor's degree is required, with a Master’s or Doctoral degree in Counseling, Psychology, or a closely related field preferred.

• A valid, active, and unrestricted Professional Counselor license in the state of New Jersey is obligatory.
