Key Responsibilities\n

• Conduct comprehensive assessments to understand the unique needs of each patient.

• Maintain ongoing communication with supervisors, care staff, and family members to track individual progress.
\n
• Create and manage a planned schedule of appointments and home visits to ensure effective patient monitoring and support.
\n
• Coordinate in-home observations and chart reviews to provide holistic assessments and inform treatment plans.

• Ensure that the implementation of the program plan is effective, continuously evaluating and adjusting strategies as necessary.

• Participate actively in Interdisciplinary Team Meetings to contribute insights and collaborate on patient care.
\n
• Document and complete monthly and quarterly progress notes to maintain records that inform treatment improvements.
\n
• Organize training sessions for staff to enhance their competencies in program implementation and documentation.
\n
• Provide follow-up support through interviews, home visits, and data evaluations to address ongoing patient needs.
\n
• Function as a liaison between the practice and the broader mental health community, advocating for evolving development goals.
\n
• Assist in designing training materials and procedures that optimize program effectiveness and staff capabilities.

Patient Population / Client Focus\n

The LPC will serve a diverse client base, including both adults and youth experiencing a variety of mental health challenges. This role particularly emphasizes the therapeutic needs within families and couples, addressing issues such as trauma, substance misuse, and relational dynamics. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Must possess an active and unrestricted Professional Counselor license in the state of Texas.

• Specialized training or experience in family therapy, couples therapy, trauma-informed care, or substance misuse is highly desirable.
