Patient Care Coordinator

Patient Care Coordinator Location: 345 E 37th St, New York, NY 10016
Work Setting: Core - On Site
Hours: 8:30AM-5:00PM Position Summary This position is responsible for accomplishing various task that contributes to the organization success and patient satisfaction. Team Providers, Medical Assistants, Nurses (RN/LPN), PSA, and echo techs averaging 21 members daily. Fast-paced, diverse team environment with responsibility for day-to-day operations, compliance, and coverage across sites as needed. Primary Responsibilities
Required Experience: 2 yearsMedical, dental, and vision insurance are available to qualified candidates who meet eligibility requirements.

- Obtain and update patient information
- Perform insurance verification
- Obtain patient signatures
- File, Fax and maintain medical records
- Schedule appointments
- Answer incoming and outgoing calls
- Check In and Check Out patients
- Manage work queues, referrals, and front desk in-basket
- Schedule specialist appointments and follow-up services
- Ensure continuity of care through scheduling and tracking systems
- Scan and assign documents to electronic charts
- Maintain HIPAA and OSHA compliance
- Crosstrain and assist at other locations as needed
- Perform other duties as assigned
