Job Description:

The registered nurse will provide professional nursing care for clients in the home health setting, covering the areas of Marion, Washington, Taylor, LaRue, Hardin, Meade, and Breckenridge Counties. The role requires Home Care Home Base charting experience and a background in home health. Responsibilities include managing a caseload of approximately 30 adult patients and meeting daily point requirements.

Responsibilities:

Case manage approximately 30 adult patients.
Complete start of care, recertifications, and daily visits.
Coordinate with the clinical manager or educator for field support.
Participate in weekend and holiday rotations as required.
Document care effectively using Homecare Homebase (HCHB).

Requirements:

1 year of experience on a Medsurg floor in a hospital setting.
Licensed in the state of Kentucky.
BLS Certification.
Experience with Home Care Home Base charting.
Scrubs are required; no color requirement specified.
