Position Overview

Gundersen Health and Wellness Clinic is seeking a dedicated and skilled Nurse Practitioner (NP) to join our Center for Advanced Cardiac Care (CACC) in Nanuet, New York. Work Environment

This position is based onsite in our outpatient office, where you will engage with a diverse patient population in a supportive and dynamic healthcare setting. Key Responsibilities

• Conduct thorough patient histories and perform detailed physical examinations to assess patient health.
• Order, interpret, and act on results from diagnostic tests to diagnose and manage both chronic and acute cardiac conditions.
• Collaboratively develop and implement goal-directed treatment plans tailored to individual patient needs, ensuring adherence to established protocols.
• Facilitate care coordination with the multidisciplinary team including physicians, social workers, and nutritionists to optimize patient outcomes.
• Educate patients and their families about disease processes, care management strategies, and transition plans post-hospital discharge.
• Maintain comprehensive medical documentation in the electronic medical record (EMR), ensuring accurate and timely updates of patient encounters.
• Participate in quality improvement initiatives and research protocols to enhance clinical practices and patient care delivery.
• Engage in daily collaboration with nursing staff and NP peers to triage patient inquiries and optimize care management strategies.
• Perform other duties as assigned, contributing to a culture of professionalism and accountability.

Patient Population / Client Focus

You will primarily work with patients suffering from complex cardiac conditions, including advanced heart failure, pulmonary arterial hypertension, and post-heart transplant care. Our practice emphasizes innovative therapies and management strategies that enhance the quality of life for these patients, including outpatient intravenous and inhaled medication therapy as well as mechanical circulatory support management.

Qualifications

• A Master's degree in Nursing with a specialized Nurse Practitioner training focused on adult cardiac care.
• Possession of an active New York State Registered Professional Nurse and Nurse Practitioner licenses.
• Current NP Board certification is required, along with certifications in BCLS, ACLS, DEA, and NPI.
• Proficiency in using electronic medical records software and general computer applications, including MS Word, PowerPoint, and Excel.

Preferred Experience

• Two years of clinical experience in an acute care or specialized hospital setting, ideally in cardiology.
• Experience managing advanced cardiovascular conditions and familiarity with outpatient therapeutic modalities.
• Demonstrated ability to work effectively within a multidisciplinary team.

Compensation and Benefits

We offer a competitive annual salary ranging from $120,000 to $220,000, commensurate with qualifications and experience.
