Your daily responsibilities will include evaluating patients in both inpatient and outpatient settings and crafting personalized treatment plans designed specifically for their needs. You will administer physical therapy interventions centering around mobility, strength, balance, and overall functionality. Collaboration is key in our practice; you will work closely with nurses, physicians, case managers, and fellow rehabilitation experts to optimize the outcomes for those in your care. Educating patients and their families on therapy goals, discharge planning, and home exercise routines will also be a crucial part of your role. Notably, meticulous documentation of evaluations, treatments, and progress in accordance with facility guidelines and regulations is essential. Engage in care team rounds and interdisciplinary conferences to foster coordinated, holistic care.

Qualifications

Candidates for this position should have a Doctorate in Physical Therapy (DPT) or an equivalent degree obtained from an accredited program. Furthermore, you must be licensed as a Physical Therapist within the state of California. CPR certification is preferred, and specialized certifications related to neurological or geriatric care will certainly be considered an asset, though not mandatory. Experience is highly valued; we prefer a candidate who has previously worked in settings treating patients with intricate medical and mobility challenges. Effective communication and teamwork skills are paramount, as is comfort with digital charting and clinical documentation systems.
