Key Responsibilities\n

• Conduct comprehensive assessments by reviewing patient health histories in preparation for initial evaluations.

• Develop tailored mental health care plans in accordance with established best practices and standards.

• Engage with patients and their families through interviews to identify educational needs and collaboratively establish treatment goals.

• Monitor and evaluate patient and family responses to treatment plans, making necessary modifications to enhance their effectiveness.

• Collaborate with physicians and interdisciplinary team members to ensure cohesive patient care and communicate status updates effectively.

• Ensure patient confidentiality in compliance with HIPAA regulations while managing all documentation meticulously in our electronic medical records system.

• Lead multiple scheduled educational workshops, focusing on “Healthy Minds” and other relevant mental wellness topics.

• Provide recommendations and referrals to additional healthcare professionals as indicated based on the patient's health history and ongoing needs.

Patient Population / Client Focus\n

In this role, you will primarily support individuals experiencing a range of mental health challenges. Our patient population includes adults and seniors, many of whom have specific health concerns such as cardiac conditions that require integrated mental health support. Qualifications\n

• Must hold a valid LMHC, LCSW, LMFT, or psychologist license in the state of Florida.

Compensation and Benefits\n

The salary for this position ranges from USD 90,000 to 104,000 annually, commensurate with experience and the number of clients seen weekly.
