Key Responsibilities\n

• Conduct thorough, ongoing assessments to determine patient needs, ensuring customized care plans are developed and implemented to address specific mental health challenges.

• Regularly communicate with supervisors and interdisciplinary team members, sharing progress updates and collaborating on treatment strategies to ensure patient success.

• Provide tentative schedules for appointments and home visits, facilitating appropriate planning for ongoing patient support and clinical assessments.

• Organize and coordinate home visits for personalized assessments, comprehensive chart reviews, and team interviews to track and enhance patient progress.

• Actively participate in Interdisciplinary Team Meetings and other relevant workshops or gatherings to contribute insights and benefit from collaborative learning opportunities.

• Complete detailed monthly and quarterly progress notes that reflect the patient's evaluations, advancements, and areas requiring further focus.
\n
• Lead training sessions designed to equip staff and team members with the necessary skills and documentation practices to implement program plans effectively.

• Conduct follow-up interviews and home visits to ensure ongoing support for clients' evolving needs and foster lasting therapeutic relationships.

• Maintain accurate and comprehensive records, ensuring that necessary information is readily accessible for team members and related parties to facilitate continuity of care.

• Act as a key liaison with the community and mental health networks, promoting Rethink’s goals and advocating for patient needs.

• Develop procedural training materials aimed at enhancing program delivery and effectiveness for patient care.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Clinical Counselor (LPCC) in California.

• Minimum of two years of hands-on counseling experience, preferably in a clinical setting.

• Demonstrated knowledge of legal and ethical standards governing mental health practices.

• Exceptional interpersonal and communication skills, with the ability to build rapport with a wide variety of clients.
