Work Environment

This role operates primarily in an onsite home care setting, emphasizing the delivery of therapeutic services directly to patients in their homes. You will work a 40-hour weekly schedule from Monday to Friday, allowing for flexibility and a work-life balance that supports personal and professional well-being. Key Responsibilities

• Conduct comprehensive evaluations, develop customized treatment plans, and implement effective therapeutic interventions in accordance with physician referrals.
• Utilize appropriate assessment tools and techniques to track patient progress and modify treatment strategies as necessary to meet goals.
• Deliver therapy for diverse patient populations including those with orthopedic, pediatric, neurologic, and sports medicine conditions.
• Demonstrate outstanding customer service, ensuring that every patient interaction reflects our core values and commitment to care.
• Collaborate with interdisciplinary teams to achieve optimal patient outcomes, utilizing evidence-based practices in your methodologies.
• Educate and consult with patients and their caregivers, empowering them with the knowledge to assist in their recovery and ongoing health management.

Patient Population / Client Focus

Your role will involve working closely with a varied clinical population, requiring you to tailor your therapeutic approach to meet the specific needs of each patient. This includes evaluating and treating individuals recovering from injuries, managing chronic conditions, and providing preventive care to enhance physical functionality. You will build rapport, trust, and understanding, ensuring patients feel supported throughout their rehabilitation journey.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program recognized by the American Physical Therapy Association (APTA) or the Commission on Accreditation in Physical Therapy Education (CAPTE).
• Current state licensure in Physical Therapy is required.
• A Basic Life Support Certification from the American Heart Association must be obtained.
• Proficient verbal and written communication skills to facilitate effective interaction with patients and team members.
• Strong organizational skills and familiarity with electronic medical records or clinic management systems.
• Ability to connect with patients on a personal level, fostering trust and enhancing the therapeutic relationship.

Preferred Experience

• Prior experience in a home health setting is highly advantageous.

Compensation and Benefits
