Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate the needs and challenges of clients, leading to the development of tailored treatment plans that address their unique situations.

• Provide comprehensive individual, group, and family therapy sessions remotely, utilizing evidence-based practices such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care approaches.

• Maintain accurate and confidential documentation of client interactions and treatment plans in compliance with New York state regulations and agency standards.

• Participate in regular case reviews and team meetings to discuss strategies, share best practices, and engage in continuing education activities aimed at professional development.

Patient Population / Client Focus\n

This role primarily focuses on patients in hospice care, including individuals managing terminal illnesses and their families. Qualifications\n
