Work Environment\n

This position is remote, allowing for flexibility in managing work hours while providing vital support to clients in their homes or care facilities. The work schedule primarily runs from Monday to Friday between the hours of 8:00 AM and 4:30 PM, with the possibility of occasional evening or weekend hours depending on client needs. As a critical member of our team, you may also be required to be on-call to address urgent situations or provide additional support as necessary.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for seniors, taking into account emotional, mental, and social factors.

• Collaborate with clients and their families to create customized care plans focused on their individual goals and needs.

• Provide counseling and emotional support, guiding clients and families through transitions such as illness, loss, and adjustments in living arrangements.

• Advocate for the rights of seniors to access appropriate community resources, ensuring they receive the support necessary for their well-being.

• Work within interdisciplinary teams composed of nurses, caregivers, and other healthcare professionals to coordinate holistic care plans.

• Perform regular client visits to monitor conditions, evaluate safety and living circumstances, and follow up on care plan effectiveness.

• Facilitate lines of communication among family members, mediating any conflicts that may arise during the caregiving process.

• Educate clients and families about available community resources, support groups, and programs to promote social engagement and enrich their lives.

• Document client assessments, care plans, progress notes, and interactions in compliance with regulatory standards and ethical practices.

• Stay updated on emerging practices, policies, and regulations relevant to social work in healthcare settings.

Qualifications\n

Compensation and Benefits\n

This position offers a competitive salary range of $60,000 to $100,000 per year, commensurate with experience and qualifications.
