Intensive Case Manager (Bilingual in Spanish)

Job Summary: The Intensive Case Manager provides intensive and comprehensive case management services in Spanish to formerly homeless residents housed in the Broadway Sansome apartments, (families and single adults) and the newly opened senior building, Philip C Chin Commons; both buildings provide supportive housing to these residents with various complex medical, mental health and substance use histories. This position requires a motivated and resourceful person, who shows sensitivity to helping address the needs of a diverse, ethnic, and formerly homeless population. This position is ideal for someone who has an interest in social work, mental health, affordable housing, and non-profit work. Status: Full-time, Nonexempt, Regular Location: North Beach neighborhood, San Francisco Hours: Monday through Friday, 9:00a.m.-5:30p.m., plus some nights and weekends Salary: $33.57 - $35.00 per hour; Comprehensive Benefits The Organization: A private non-profit founded in 1977, Chinatown Community Development Center believes in a comprehensive vision of community. What You Will Do Complete regular outreach and provide intake and assessments to new residents inviting them to access services as needed Offer on-going supportive services with residents, including assistance setting up medical and health appointments, employment services, and creating service plans Utilize clinical skills and practices including de-escalation, mediation, and psycho-educational individual and supportive services and groups, if applicable Deal with housing stability issues and mediation with property management Help organize and attend interdisciplinary and interagency meetings and events with residents and community partners Job Responsibilities Conduct intakes and assessments to establish strengths, skills, needs, and goals to formulate individualized action plans for residents Provide case management and counseling services to establish goals, support individualized action plans, and track resident progress towards meeting goals Provide assistance and referrals to help residents obtain/maintain benefits, increase income, and connect with community resources Facilitate individualized counseling on various topics including mental health, substance use, and home maintenance, and groups, if requested Work in collaboration with Property Management to orient residents to and provide ongoing support regarding the lease, lease addendums, and house rules Assist residents in communicating with and responding to Property Management notifications Provide conflict mediation between residents and between residents and Property Management Work with the sites’ ICMs and Resident Services Coordinators to provide/coordinate services, events and activities that strengthen families and seniors Attend weekly Property Management and monthly resident and staff meetings Perform health and wellness checks as needed Maintain resident files on all services provided, including intakes, needs assessments, service plans, referrals, and outcomes Track resident services data in multiple databases on a regular basis Complete/maintain service reports, grant reports, activities budgets, program files, and other paperwork Provide information for and/or assist with writing grant applications Conduct resident surveys to measure program goals and outcomes Assist Resident Services team and other staff organization at events Attend various internal and external meetings and trainings Other Embody organization values (respect & compassion, empowerment, teamwork) Contribute to a safe and pleasant work environment Follow policies, procedures, and safe work practices Other duties as assigned What you bring General understanding of, and sensitivity to, issues related to homelessness, substance use, mental health, and aging amongst the very low-income population Strong analytical, problem-solving, and follow-through skills Ability to collaborate with other social service agencies Ability to maintain confidentiality of information Capacity to operate independently, take initiative, and exercise independent judgment Ability to remain professional in stressful situations Ability to work effectively with culturally diverse populations Experience working on an interdisciplinary team Education and Experience (Minimum Qualifications) Bachelor’s Degree in Social Work or related field Three years of experience providing intensive case management services and referrals to low-income individuals and families with mental health and/or substance use issues One year of experience working with families Bilingual and biliterate in English/Spanish Knowledge of, and sensitivity to, the needs of low-income populations Experience with crisis de-escalation and the harm-reduction model Experience with support or psychoeducational groups Preferred Qualifications Master of Social Work or related field Knowledge of public benefits and how to screen for benefits eligibility Experience providing services in a Permanent Supportive Housing environment and working in conjunction with on-site Property Management to promote housing stabilization/retention for low-income residents Familiarity with the resources available in the neighborhoods and surrounding neighborhoods of the work sites Prior experience measuring program outcome
