Work Environment

This onsite role involves working directly within the homes of patients throughout Fort Lauderdale, requiring adaptability and a strong commitment to patient rapport. Our therapists are equipped with mobile tools to ensure seamless service delivery, including access to digital documentation systems and telehealth coordination when needed. Key Responsibilities

• Conduct initial evaluations for patients in their homes to assess needs, limitations, and goals related to mobility and function.
• Develop comprehensive, individualized treatment plans incorporating therapeutic exercises, manual therapy techniques, and modalities to alleviate pain and promote mobility.
• Implement and adjust treatment strategies based on patient progress and feedback, regularly documenting outcomes in electronic health records.
• Engage patients and their families in their treatment plans, providing education on exercises, use of any prescribed assistive devices, and safe mobility practices.
• Regularly re-evaluate patient progress, reassessing goals, and modifying treatment plans accordingly to optimize outcomes.
• Collaborate with interdisciplinary teams including physicians, nurses, and occupational therapists to share insights and improve patient care strategies.
• Participate in ongoing professional development by staying informed on best practices, attending workshops and engaging in professional societies.

Patient Population / Client Focus

As a Physical Therapist at Slate Care Solution, you will work with a diverse patient population ranging from pediatric patients to elderly individuals. Our focus extends to those with musculoskeletal injuries, neurological disorders, post-surgical rehabilitation needs, and chronic illness recovery. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state license in Physical Therapy.
• Excellent verbal and written communication skills necessary for patient interactions and documentation.
• Proficient in computer skills and familiarity with electronic health record systems.
• Strong organizational skills and a flexible approach to changing needs within the patient care environment.

Preferred Experience

• Two years of experience in a clinical setting, with prior home care experience being an advantage.
• Current CPR certification.
• Doctorate of Physical Therapy (DPT) is strongly preferred.

Compensation and Benefits
