Key Responsibilities

• Develop individualized treatment plans for patients, effectively communicating these to residents, caregivers, and other healthcare providers involved in their care.
• Order and interpret lab tests and diagnostic procedures to ensure accurate assessments and modifications to treatment plans when necessary.
• Maintain comprehensive and compliant patient health records using electronic medical records (EMR) systems, ensuring timely and accurate documentation.
• Coordinate with local and regional operations and clinical leadership to address operational goals and improve patient care outcomes.
• Advocate for value-based care initiatives, enhancing the quality and efficiency of healthcare delivery.
• Deliver exemplary customer service to foster high levels of patient satisfaction and retention.
• Practice in accordance with the professional scope, standards, and ethical guidelines as stipulated by relevant nursing boards.
• Collaborate effectively with primary care providers and other members of the interdisciplinary care team at the facility to ensure comprehensive care.
• Conduct all regulatory required visits, including monthly, quarterly, and annual evaluations, alongside acute visits as required.
• Engage in family conferences to discuss care plans, providing education and support for advanced care planning.
• Address any pharmacy-related inquiries or recommendations from pharmacists concerning patient medications.
• Ensure consistent presence during working hours at assigned facilities to meet resident care needs through direct face-to-face interactions.
• Assess, diagnose, and manage both chronic and acute health conditions, offering health education and guidance to promote wellness.

Qualifications

• Must hold a Bachelor’s Degree in Nursing or a related field.
• A National Certification from either AANP or ANCC is required.
• Active, unrestricted state license to practice as a Nurse Practitioner.
• An active DEA license is necessary.
• Must possess authorization to work in the United States.
