Adult-Gerontology Nurse Practitioner - Full-Time, Onsite Care Specialist

Position Overview

Work Environment

This role requires working onsite within an outpatient practice, where you will be an integral part of a collaborative care team. Your colleagues will include physicians, nurse practitioners, physician assistants, medical assistants, dedicated medical scribes, and care managers, all dedicated to enhancing patient outcomes. Key Responsibilities

• Deliver outstanding primary care by performing comprehensive assessments, diagnosing conditions, and developing evidence-based treatment plans.
• Conduct routine office visits and manage acute health issues, effectively utilizing clinical judgment and evidence-based practices.
• Administer annual wellness visits and health risk assessments, ensuring accurate and thorough documentation of patients’ health statuses.
• Coordinate care with specialists, laboratories, and healthcare agencies to facilitate comprehensive patient management.
• Take an active role in population health initiatives, influencing the healthcare decisions that benefit our patient community, such as ensuring adherence to preventive care guidelines.
• Engage in phone triage and outreach to support patient care continuity, resolving healthcare inquiries, and guiding patients through their health journeys.
• Educate patients on their health conditions, empowering them to understand their care plans and treatment options.
• Participate in promotional activities to enhance community awareness about our services and improve patient accessibility.
• Conduct home visits as needed to provide care to patients who are unable to attend clinic appointments.
• Perform additional duties as assigned, maintaining close collaboration with the Medical Director and operational leadership.

Patient Population / Client Focus

Your community-based practice emphasizes primary care for older adults, focusing on keeping them healthy and enabling them to enjoy life. This includes managing chronic conditions, preventive care, and health education that empower patients to maintain independence and high quality of life.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or graduation from an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license and an active DEA license.
• Demonstrated experience in primary care, especially within internal medicine, geriatrics, or family medicine.
• A solid understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• A demonstrated passion for teamwork and collaboration across interdisciplinary lines, essential for providing comprehensive patient-centered care.

Preferred Experience

• Experience in a concierge medicine or home care setting is highly valued.
• Familiarity with electronic health records and practice management tools that enhance patient care efficiency.

Compensation and Benefits

We offer a competitive salary range between USD 120,000 to 160,000 annually, commensurate with experience and qualifications.
