Key Responsibilities

• Conduct thorough initial patient screenings, including taking detailed histories and performing physical examinations to assess patient health.
• Continuously update and maintain accurate patient records, checking for quality and completeness at every appointment to ensure the highest standards of care.
• Interpret test results, analyze data, and determine the necessity for follow-up appointments and additional treatment options.
• Deliver comprehensive care while adhering to company metrics regarding productivity and quality, ensuring that every patient receives optimal treatment.
• Act as the primary health resource for patients, providing ongoing information, counseling, and guidance, particularly for those with chronic or critical conditions.
• Educate patients on topics including health maintenance, prevention of diseases, and various treatment options, with a focus on harm reduction techniques.

Patient Population / Client Focus

Your primary focus will be on a diverse population comprising students and their families, each with unique healthcare needs. You will be responsible for delivering care to individuals ranging from children to young adults, emphasizing disease prevention, health promotion, and management of acute and chronic conditions. This role requires effective communication and rapport-building skills to engage with patients and their families effectively.

Qualifications

• Certification as a Family Nurse Practitioner is required.
• A current and valid Family Nurse Practitioner license in New York is essential.
• A Master's degree in Nursing or a Bachelor’s degree along with an approved supplemental education program, allowing for the provision of acute/primary healthcare services.
• A minimum of two years of nursing experience in a relevant specialty, emphasizing family medicine.
• Demonstrated proficiency in performing physical examinations, diagnosing common illnesses, and effectively managing chronic conditions.
• Exceptional communication skills to build strong relationships with patients, families, and healthcare colleagues.

Preferred Experience

• Experience working in a school-based health setting or with pediatric populations is highly desirable.
• Proven track record of managing a diverse patient caseload and implementing comprehensive care plans.
• Understanding of public health principles and the ability to participate in community health initiatives.

Compensation and Benefits

This position offers a competitive salary ranging from $150,000 to $160,000 annually, reflecting your experience and expertise.
