Work Environment

The nurse practitioner will work onsite at our community-centered clinic, which prides itself on its collaborative atmosphere. The facility focuses on home care as part of our service offerings, allowing practitioners to engage with patients in a setting familiar to them, enhancing their comfort and health outcomes. The regular work schedule is set from 9:00 a.m. to 6:00 p.m., with occasional evenings until 8:00 p.m. Additionally, flexibility to participate in community outreach events and health fairs on weekends is expected, reflecting our commitment to community engagement.

Key Responsibilities

• Conduct comprehensive health assessments, performing detailed physical exams and screenings to evaluate the health status of individuals and families.
• Diagnose and manage acute and chronic medical conditions, formulating care plans that include preventive health measures and promoting wellness.
• Work collaboratively with community organizations to design and implement relevant health education programs addressing prevalent health issues.
• Deliver culturally sensitive care and advocate for patients' rights, ensuring that their healthcare needs are met with dignity and respect.
• Coordinate patient referrals and follow-up care, enhancing continuity of care and ensuring that patients receive necessary interventions.
• Maintain accurate and up-to-date medical records in compliance with healthcare regulations, fostering effective communication within the care team.
• Participate in outreach activities, including health fairs and community events, to provide health screenings and increase awareness of available services.

Qualifications

• Current certification and licensure as a Nurse Practitioner in the state of New York.
