Work Environment

This position is primarily onsite within an Assisted Living Facility, providing a welcoming and supportive environment for residents. Key Responsibilities

• Conduct thorough assessments of patients by obtaining and documenting comprehensive health histories and performing physical examinations.
• Diagnose and manage both acute and chronic health conditions, tailoring treatment plans to meet individual patient needs.
• Order, interpret, and follow up on laboratory tests and diagnostic imaging as part of the ongoing evaluation of patient health.
• Implement pharmacologic and non-pharmacologic interventions while ensuring adherence to New York State practice regulations.
• Set measurable patient outcomes and goals, evaluating the effectiveness of treatment protocols and modifying them as necessary.
• Educate patients and their families on health promotion, wellness strategies, and anticipatory guidance to foster empowerment and proactive health management.

Patient Population / Client Focus

In this role, you will predominantly serve adult residents of the Assisted Living Facility, who often present with multiple chronic conditions requiring comprehensive management. Your expertise in Internal Medicine will be essential in delivering appropriate care that evaluates both medical and psychosocial aspects of health, enhancing the overall well-being of our residents.

Qualifications

• A Master’s degree in Nursing with a focus on Nurse Practitioner is required.
• Current New York State Registered Professional Nurse and Nurse Practitioner licenses.
• Certification by a recognized Nurse Practitioner Board.
• Basic Life Support (BCLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
• Strong clinical skills with excellent interpersonal and communication abilities—capable of articulating complex medical information to patients and families.

Preferred Experience

A minimum of three years experience as a Nurse Practitioner in an Internal Medicine setting is preferred. Candidates with a track record of managing a diverse range of patient cases, especially in geriatrics or assisted living environments, will stand out.

Compensation and Benefits
