Work Environment

As a Family Nurse Practitioner, you will work onsite in Southampton, a vibrant community in Long Island known for its beautiful landscapes and close-knit neighborhoods. Your daily schedule will typically run from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. You may also participate in community outreach events and health fairs during weekends, further solidifying our commitment to community engagement. This role promises approximately 40 hours of work per week in a supportive and collaborative environment.

Key Responsibilities

• Conduct comprehensive health assessments, including screenings and diagnostics, for diverse individuals and families.
• Manage acute and chronic medical conditions, while emphasizing preventive care, wellness promotion, and patient education.
• Collaborate with community organizations to develop and implement health education programs tailored to local needs.
• Advocate for culturally sensitive care, ensuring patients' rights are upheld and their healthcare access is maximized.
• Coordinate referrals and arrange follow-up care, maintaining continuity and quality of care for your patients.
• Maintain meticulous and accurate medical records in compliance with healthcare regulations and organizational policies.
• Participate in community events to provide health screenings and raise awareness of important health issues.
• Stay informed about community health trends and evidence-based practices to ensure the highest standards of care.

Patient Population / Client Focus

Your practice will focus on a diverse patient population, including underserved individuals and families who benefit significantly from personalized healthcare approaches. By engaging in house calls and concierge services, you will have opportunities to build lasting relationships with patients, ensuring they receive attentive and responsive care tailored to their individual health needs.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited institution.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Possession of a DEA number and NPI number is required.

Preferred Experience

• Minimum of 3-5 years of clinical experience in a primary care or family medicine setting; experience in community health or public health is preferred.
• Strong clinical assessment, diagnostic skills, and proficiency in using Electronic Health Records (EHR) systems.
• Exceptional communication and interpersonal skills, with the ability to connect with diverse populations.
• Capability to work independently while remaining an integral part of a collaborative healthcare team.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $130,000 per year, commensurate with experience and qualifications. Our comprehensive benefits package includes:
