Work Environment

This position is an onsite role based in Southampton, New York, where you will have the opportunity to work directly with patients in a home care setting. Key Responsibilities

• Conduct thorough health assessments and screenings of patients, utilizing evidence-based practices to diagnose and treat acute and chronic medical conditions.
• Manage medication plans, ensuring prescriptions are tailored to meet each patient's individual needs, while consistently monitoring outcomes for effectiveness.
• Participate in community outreach events and health fairs to elevate health awareness and provide essential health screenings.
• Coordinate and facilitate referrals for specialized care, ensuring seamless transitions and continuity of care throughout the patient’s healthcare journey.
• Engage with community organizations to develop and implement health education programs that address the needs of diverse populations.
• Maintain meticulous and up-to-date medical records compliant with regulatory standards and best practices in healthcare documentation.
• Provide culturally sensitive care to residents, advocating for their rights and increasing their access to essential healthcare services.

Patient Population / Client Focus

In this role, you will primarily focus on patients requiring cardiology expertise, particularly those within skilled nursing and home care settings. Your compassionate approach will help you connect effectively with patients facing chronic health challenges, including heart disease and related conditions, ensuring that they receive tailored support in managing their health and wellbeing.

Qualifications

• A Master’s degree in nursing from an accredited institution, qualifying you for advanced nursing practice.
• An active nursing license in the state of New York, along with current certification as a Nurse Practitioner.
• A minimum of three years of clinical experience, with a preference for candidates who have worked in community health or public health settings.
• Strong interpersonal and communication skills, capable of effectively interacting with diverse patient populations and healthcare teams.
• A demonstrated commitment to social justice, health equity, and advocacy for vulnerable communities.

Preferred Experience

• Prior experience working within a cardiology practice or similar clinical environment.
• Hands-on experience in home healthcare or with patients in skilled nursing facilities.
• Experience in developing patient education materials and leading health-related workshops.

Compensation and Benefits
