The work schedule offers operational hours from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m., supporting flexible patient access. You may also engage in weekend community outreach events and health fairs, further solidifying our commitment to proactive health initiatives.

Key Responsibilities

• Conduct detailed health assessments and screenings for patients of all ages, identifying specific health needs and potential risks.
• Diagnose and manage acute and chronic medical conditions while promoting preventive care strategies tailored to individual and family health needs.
• Work closely with local organizations to develop and implement effective health education programs that address community-specific health issues.
• Advocate for patient access to healthcare services, ensuring culturally respectful interactions and supporting their healthcare rights.
• Coordinate patient referrals and follow-up care seamlessly, ensuring comprehensive management of each patient’s health plan.
• Maintain accurate, detailed medical records adhering to HIPAA and regulatory standards to promote safety and quality care.
• Engage in community outreach efforts and health fairs, providing screenings and health information to enhance community awareness.
• Stay informed about local health trends and evidence-based practices to continually improve service delivery.

Patient Population / Client Focus

This role primarily focuses on families and individuals within the East Hampton area, particularly underserved populations who face various healthcare disparities. Your work will directly impact community health by addressing both preventive and chronic health issues, promoting wellness, and ensuring access to necessary health services. The goal is to empower patients through education and tailored health management plans.

Qualifications

• Active certification as a Nurse Practitioner, with a strong clinical foundation in Family Practice.
• Possession of a current state nursing license for New York.
• A Master’s degree in nursing from a nationally accredited program.
• Minimum of three years clinical experience, preferably within community or public health settings.
• Demonstrated interpersonal and communication skills, with a focus on working effectively with diverse populations.

Preferred Experience

Compensation and Benefits
