Flexibility in scheduling is paramount to ensure we meet our clients where they are and provide timely support.

Key Responsibilities\n

• Conduct comprehensive assessments of the psychosocial requirements of senior clients, considering their emotional, mental, and social contexts.

• Develop and implement individualized care plans in collaboration with clients and their families to meet unique goals and preferences.

• Deliver counseling services and emotional support, aiding clients through transitions such as bereavement, relocation to assisted living, or adapting to chronic conditions.

• Advocate for clients' rights and facilitate their access to essential community resources, promoting their independence and enhanced quality of life.

• Engage in interdisciplinary teamwork with healthcare professionals, such as nurses and caregivers, to provide holistic care solutions that improve client outcomes.

• Regularly visit clients to assess their living conditions, monitor emotional states, and address any safety or health concerns.

• Guide family members in effective communication strategies, conflict resolution, and collaborative decision-making pertaining to the care of their loved ones.

Compensation and Benefits\n

The salary for this position ranges from USD 60,000 to 100,000 per year, dependent on experience and credentials.
