This full-time position is based remotely while primarily serving our Hospice Center located in beautiful Salinas, California. The role offers a competitive salary ranging from USD 115,000 to 130,000 annually, commensurate with experience and qualifications.

Key Responsibilities\n

Your day-to-day responsibilities will include:

• Conducting comprehensive clinical assessments to identify patient needs and appropriate interventions.
\n
• Delivering short-term psychotherapy and facilitating group therapy sessions for patients and their families, addressing issues related to grief, loss, and chronic illness.

• Providing consultation and support for counseling needs, ensuring patients receive tailored care plans.
\n
• Planning and executing outreach activities to engage with the community and educate on mental health resources available through our hospice care.
\n
• Participating in ongoing training and professional development to enhance your skills and stay current with best practices in social work.
\n
• Conducting crisis assessments and interventions both in-person and online, ensuring immediate support is accessible to those in acute distress.
\n
• Practicing in accordance with legal and ethical standards while assessing suicidal concerns, arranging for hospitalization when necessary, and developing treatment plans that include appropriate psychotherapeutic interventions or referrals for long-term care.

Patient Population / Client Focus\n

The Licensed Clinical Social Worker will work primarily with a diverse population of patients within the hospice setting, including adolescents and adults facing chronic illnesses and end-of-life issues. You will engage with patients who require empathetic support for emotional challenges and facilitate discussions about their healthcare goals and preferences within a multidisciplinary team.

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master’s Degree in Social Work from an accredited institution.

• Current Licensed Clinical Social Worker (LCSW) credentials, with consideration for fully licensed Licensed Mental Health Counselor (LMHC) candidates.

• A minimum of 3 years of post-master's clinical experience, particularly in case management within behavioral health settings.
\n

Preferred Experience\n

• Experience working with adolescents or college-aged individuals managing treatment in a clinical setting.

• Background in hospice care or similar end-of-life service environments is highly desirable.

• Familiarity with virtual therapy tools and platforms to support telehealth services.
