Key Responsibilities\n

• Coordinate with Reentry Social Workers to develop and implement effective discharge planning services tailored to individual patient needs.

• Complete required statistical reports and documentation accurately to maintain compliance and enhance program effectiveness.
\n
• Engage with the Department of Correction to secure access to patients, ensuring they receive appropriate housing and treatment dispositions.
\n
• Collaborate with different institutional programs to deliver a cohesive continuum of care; develop and facilitate group therapy sessions in mental health observation units.
\n
• Conduct thorough mental health evaluations, including crisis intervention and follow-up care planning that aligns with patient needs.
\n
• Participate actively in clinical supervision with a Clinical Supervisor on a weekly basis to support professional growth and case management strategies.
\n
• Lead treatment planning and case conferences for patients, integrating feedback from various healthcare professionals involved in patient care.

• May supervise and mentor social work interns or student volunteers, enhancing their education and skills in a clinical setting.

• Maintain effective communication with all stakeholders to ensure consistency in patient care.”

Patient Population / Client Focus\n

This role primarily involves working with children, teenagers, and families who are navigating the emotional and psychological challenges associated with end-of-life care. Compensation and Benefits\n

NewBetterYou Healthcare Services, LLC offers a competitive salary ranging from $80,000 to $122,000 annually for this full-time position, commensurate with experience and qualifications.
