Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify patients’ emotional, social, and financial needs.

• Collaborate with interdisciplinary medical teams to develop and implement holistic care plans that address the full range of patient needs.

• Provide individual and group counseling to patients and their families, focusing on adjustment, grief, and coping with terminal illnesses.

• Facilitate crisis interventions, using effective techniques to provide immediate support to patients facing acute psychological distress.

• Coordinate with community organizations to ensure continuity of care and arrange for post-discharge services.

• Develop and implement thorough discharge plans, connecting patients and families with relevant community resources.

• Maintain accurate and timely documentation in compliance with hospital policies and state regulations.

• Participate in hospital committees and contribute to discussions regarding social work-related matters.

Compensation and Benefits\n

We offer a competitive salary range of $70,000 to $100,000 annually, depending on experience and qualifications, alongside a sign-on bonus.
