Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments that address the mental health, substance abuse, and medical needs of clients, ensuring that the evaluations are culturally and age-appropriate.

• Provide therapeutic services to individuals, couples, and families, including counseling sessions, intake assessments, treatment planning, crisis intervention, and referrals for substance use treatment when necessary.

• Maintain detailed electronic documentation of client progress, implementing appropriate mental health evaluations, progress notes, and suicide risk assessments as needed.
\n
• Act as a liaison to facilitate social work support, connecting patients to local resources and community agencies, such as medication assistance programs and additional support services.

• Collaborate with our Quality Improvement teams to design and implement data collection processes that enhance care quality and service delivery.

Patient Population / Client Focus\n

This role focuses on providing care in a hospice environment, which requires sensitivity to the unique challenges faced by patients with terminal illnesses and their families. Your contributions will be key in making the end-of-life experience as comfortable and fulfilling as possible, with a strong emphasis on mental wellness and emotional support.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current and valid state license as a Licensed Clinical Social Worker (LCSW) is mandatory.

• A minimum of three (3) years of post-licensure clinical experience in a mental health setting is required.

• Proficient understanding of counseling techniques, crisis intervention strategies, and the ability to make informed judgments related to direct patient services.

Compensation and Benefits\n
